Below is a list of OFA applications for submission of dogs and cats to OFA databases. The applications are in Acrobat format. The top section may be filled out online, printed, and taken to your local vet to complete and send to the OFA.

Note that these forms are updated first online. Your vet may have older versions of the forms, or older card applications. Please make sure your vet has the most updated material to submit applications.

Clinics, Kennels and Breed Clubs may request preprinted applications and brochures to be mailed to them.
